Employer Qualifications​

  • Up to 250 (for profit) or 500 (non-profit) employees

  • Offer a qualified benefit plan

  • Contribute at least 25% of qualified-employee premiums

Employee Qualifications​

  • Oklahoma Resident between ages 19-64

  • Annual gross household income within ESI guidelines

  • Contribute up to 15% of premium cost for self and qualified dependents

Application
Process

EMPLOYERS

  • Work with us to find a Qualified Benefit Plan

  • C&R will complete the online Employer Application through the Employer Portal

EMPLOYEES

  • Get Application Letter from employer

  • Complete the online application, following the directions in the application letter

ONGOING SUPPORT

  • C&R will upload monthly health care invoice to Employer Portal

  • C&R will monitor employee eligibility status & keep employer informed
