Blue Cross Blue Shield of Oklahoma 1095-B forms are available now in Blue Access for Members for all employees on a fully-insured group medical plan. Members can also request a paper form by calling the number on their ID card or by visiting the BCBS Help Center.

  • Does BCBSOK mail 1095-B forms?

    • The IRS no longer requires insurers to automatically mail 1095-B forms to members. However, a member may request a copy of their form be mailed to them. BCBSOK only mails forms to individual members upon request and is not able to print forms for an entire group.

  • Why doesn't BCBSOK handle 1095-C reports for groups?

    • 1095-C reporting is used for 2 purposes:

      • Self-funded groups use it to report on their employees' minimum essential coverage, and they are responsible for sending the 1095-C forms to their employees. BCBSOK does not have the necessary data.

      • Applicable Large Group employers-both Fully Insured and Self-funded groups-are required by the IRS to report who they OFFERED coverage to, which BCBSOK does not know.
